Enrollment Information

Thank you for choosing Sun Canyon
The following is needed to enroll a student at Sun Canyon:

1.       Completed Registration Forms (see below)
2.       Birth Certificate
3.       Immunizations
4.       Current Proof of Address (Utility bill, home or rental contract, insurance bill, etc.) *If you are living with another family you will need to bring in their current proof of address along with a notarized letter stating that you live with them.  Blank forms are available in the front office.
5.       Parents ID


Each student attending public school in Arizona must have on file in his/her school verifiable documentation of immunization information. The law requires written documentation which indicates the student's name, date of birth, month, day, and year of all doses received, and the name of the health care provider who administered the vaccine.
